Arizona Board of Nursing Renewal Fees and Penalties

Arizona law specifies the maximum amount the Arizona Board of the Nursing may charge nurses for applications, renewals, certifications, clerical work, fees and penalties.  The amounts for a select number of activities are as follows (all values are current as of 1/1/2011):

Initial Application

Registered Nurse Practitioner – $150.00
Clinical Nurse Specialist – $150.00
School Nurse Certification – $75.00
Registered Nurse – $150.00
Practical Nurse – $150.00
Prescribing Medication – $150.00
Certified Nursing Assistant – $50.00

Application for Renewal (before expiration)

Registered Nurse Practitioner – $150.00
Clinical Nurse Specialist – $150.00
School Nurse Certification – $50.00
Registered Nurse – $160.00
Practical Nurse – $160.00
Certified Nursing Assistant – $50.00
Prescribing Medication – $20.00

Clerical Work

Issuing a duplicate license or certificate – $25.00
Copying a nursing program transcript – $25.00
Copying records, documents, letters, minutes, applications and files – $.50 a page
Processing fingerprint cards – $50.00

The licensure/certification process can be a lengthy and expensive process, so make certain to give yourself enough time to meet the deadlines set by the Arizona Board of Nursing.  Failing to submit the correct fee amounts or meet the stringent deadlines could lead to fines or the loss of your license.
